Childcare and children's social care health declaration forms
Health declaration forms for childminders and people who work in childcare or children's social care.
Applies to England
Introduction
You will need to complete a health declaration form before you register with Ofsted to look after or care for children.
Please complete a form if:
- you’re applying to register as a childminder or childminder without domestic premises on the Early Years
RegisterRegister. Please select childminder on the health declaration form - you’re registering as the manager of childcare on domestic premises
- you’re applying to register as a social care establishment, agency or manager
- Ofsted has asked you to complete it because we need more information (for example, if you live with a childminder or if you own a nursery)

Social care applications
From 1 April 2024 a health declaration, signed by your GP, is no longer required for children’s social care applicants. If you are a social care applicant, you must declare your current health using the children’s social care health form. You do not need to send the form to your GP as part of your application to be registered as or connected to a children’s social care provider.
If you need any help completing the form, please telephone us on 0300 123 1231 or email enquiries@ofsted.gov.uk.
