Childcare and children's social care health declaration forms

Health declaration forms for childminders and people who work in childcare or children's social care.

Introduction

You will need to complete a health declaration form before you register with Ofsted to look after or care for children.

Please complete a form if:

  • you’re applying to register as a childminder or childminder without domestic premises on the Early Years RegisterRegister. Please select childminder on the health declaration form
  • you’re registering as the manager of childcare on domestic premises
  • you’re applying to register as a social care establishment, agency or manager
  • Ofsted has asked you to complete it because we need more information (for example, if you live with a childminder or if you own a nursery)

Social care applications

From 1 April 2024 a health declaration, signed by your GP, is no longer required for children’s social care applicants. If you are a social care applicant, you must declare your current health using the children’s social care health form. You do not need to send the form to your GP as part of your application to be registered as or connected to a children’s social care provider.

Completing the health declaration form

All early years applicants must complete this form.

  1. Fill in section A, the relevant part of section B and the declaration.

  2. Contact your GP to fill in section C. GPs may charge a fee for doing this.

  3. Either scan or photograph the document and send with your unique reference number (URN) to enquiries@ofsted.gov.uk.

Completing the children’s social care health form

All children’s social care applicant’s must complete this form.

  1. Fill in section A, section B and the statement of self-declaration.

  2. Send the form to enquiries@ofsted.gov.uk.

Your information

This information remains valid for 6 months from the date of your GP’s signature (health declaration form) and/or your signature (health self-declaration form). Please send this to Ofsted without delay.

It is an offence to make a statement that you know is false or misleading as part of a registration application. Please answer this form truthfully. We will not necessarily refuse your registration based on current or previous health problems.
