Updated information on risk assessment in nurseries, schools and other childcare settings (section 4.1).

Changed to using diagnosis date (definition included) instead of onset date to define close contacts and the 10-day post-exposure period. Clarified actions required within 10 days of exposure and for the 30-day surveillance period, and that actions should be undertaken for both confirmed and probable cases. Added Clarithromycin as a second line choice for chemoprophylaxis.
